유방 상피내암에서 국소 치료 이후에 전신항암치료를 하지 않으며,
HER2-directed therapy도 제한적이지만
ER 양성 환자에게 향후 유방암 재발 위험을 낮추기 위해 호르몬 치료를 권고합니다.
상피내암 (제자리암, DCIS)에서 전신 치료는 invasive breast cancer의 위험을 낮춥니다. 이와 같은 환자에서 항암치료는 역할을 하지 못하는데 전이 병변 위험이 낮고 예후도 좋기 때문입니다.
hormone receptor-positive DCIS에게 전신 치료로 호르몬 치료를 제안합니다. DCIS 치료에서 human epidermal growth factor receptor 2 (HER2)-directed therapy에 대한 현재 적응증은 없습니다.
Is there a role for HER2-directed therapy?
현재 데이터는 DCIS에서 human epidermal growth factor receptor 2 (HER2)-directed therapy의 사용을 지지하기에 충분하지 않습니다. American Society of Clinical Oncology/College of American Pathologists guidelines과 일치하게 DCIS에서 HER2 testing을 권고하지 않습니다. Invasive breast cancer와 달리 DCIS는 종종 HER2를 표현하는데 708건의 malignant ductal lesions (59 with DCIS, 649 with invasive disease) 연구에서 HER2 양성 비율은 각각 56%, 15%이었습니다.
Endocrine therapy
국소 치료 이후에 이후 암 발생 위험을 줄이기 위한 호르몬 치료 여부 결정은 호르몬 수용체 상태와 국소 치료 선택에 따라 달라집니다. ER에 대한 검사는 향후 유방암 위험을 줄이기 위한 호르몬 치료의 잠재적 이득을 결정하기 위해 권고되며 PR에 대한 검사는 선택적으로 고려됩니다.
1. For women with ER-positive DCIS who have not undergone a bilateral mastectomy, we suggest endocrine therapy rather than observation, and both tamoxifen and the aromatase inhibitor anastrozole are reasonable. Endocrine therapy reduces recurrence rates, but has not been shown to improve survival. The data to support this recommendation are discussed below.
2. We do not routinely utilize tamoxifen as chemoprevention for women with ER-negative DCIS given that the benefit of endocrine therapy does not reduce the risk of recurrence in this population. Some women may opt to take tamoxifen, however, to decrease the risk of developing a new endocrine receptor-positive DCIS or breast cancer.
3. In women having bilateral mastectomies for DCIS, the risks of adverse effects from tamoxifen outweigh any potential benefit for risk reduction.
